Are you a clinical strategist who excels at navigating the complexities of the healthcare journey? Abu Dhabi Health Services (SEHA) is seeking a highly organized and empathetic Nursing Case Coordinator to serve as the primary architect of the patient experience across the care continuum.

In this role, you will move beyond bedside care into a critical leadership position focused on “joined-up” healthcare. As a Case Coordinator, you are the bridge between patients, families, and multidisciplinary medical teams.Your mission is to eliminate barriers to recovery, ensuring that every patient moves through the system—from admission to home care—with efficiency, dignity, and a personalized plan for success.

Core Responsibilities & Impact

• Care Continuum Oversight: Manage the end-to-end journey of the patient, ensuring that transitions between departments or facilities are seamless and clinically sound.

• Patient Advocacy: Act as the primary voice for the patient, ensuring their goals, cultural needs, and medical requirements are prioritized by the multidisciplinary team.

• Strategic Care Planning: Develop, implement, and periodically review comprehensive patient care plans that align with quality and efficiency standards.

• Multidisciplinary Leadership: Facilitate communication between physicians, therapists, insurance providers, and social workers to optimize service delivery.

• Resource Optimization: Identify and secure necessary resources (medical equipment, home health services, or follow-up appointments) to prevent delays in discharge and reduce readmission rates.

• Outcome Analytics: Monitor patient progress and service efficiency to ensure the highest standards of the Abu Dhabi healthcare framework are met.

Candidate Requirements

• Education: A Bachelor’s Degree in Nursing from an accredited institution.

• Experience: Proven clinical experience, with specific background in Case Management or Care Coordination preferred.

• Licensure: Valid nursing registration and eligibility for a DOH (Department of Health – Abu Dhabi) license.

• Soft Skills: Exceptional interpersonal, negotiation, and conflict-resolution skills.

• System Knowledge: Familiarity with the UAE healthcare system and insurance processing is a significant advantage.

• Attributes: Highly organized, detail-oriented, and a commitment to “Patient-First” service delivery.
